If you've made sure they're already level range, also keep in mind there is currently an issue with many new item drops and some old ones such as medals of honor. The items themselves disappear after about 30 seconds, instead of the normal 5 minutes. This means unless you're keeping up with drops as they come, you could more easily miss them.
