that salty bandaid guys said most of teh stuff, but if you are really low on health and surrounded by boxes but no hp thing, i justlike to pick them up cuz there is a chance of invinviblity, att, def, or health and without hp i will die in like a hit anyways so might as well pick it up

oh yeha and never walk into someone elses's summons or smoke for shadowers
You seem to be doing pretty good, there's just a few little things you're doing that you could improve on.
#1: You neglect shield buffs, These actually boost your defense considerably, just like how their counter part (Fists) immensely increase your damage output. Don't risk your life for these, but if you happen to see one, don't hesitate to pick it up.
#2: If you pick up a box and turn into a monster, keep picking up boxes instead of running away from your enemies (or letting them kill you). Picking up boxes can activate extremely useful affects like invincibility (Probably the best thing you can get), and status ailments that can potentially cripple your foes; there's also the risk of being transformed again; but hey, you're already transformed, no big deal right? Also, if you're about to die (Like 20% hp or something) picking up boxes can potentially save you.
#3: You don't seem to have all of your skills that have a cooldown on your quickslot; you should put all of them on your quickslot (Regardless of how useless they are) on there, so long as you use it; this enables you to see when you can use your skills.
#4: Play on the larger resolution so you can see more.
#5: Press the "0" button on your numpad to pick up items while you're charging big bang.

I like the way you utilize Big bang and charge it when no ones around.
The way you wait for the right moment to pick up fists instead of just picking them up on sight is also impressive, this allows you to not squander the short time the fist buff is active.

That's what I derived from watching 2 minutes of your vid.
I'm too lazy to watch the full 10 minutes xD.

Anywho, it looks like you have some serious skill Dean, keep up the good work.
Already said it in my first video, I needed to play on a smaller resolution so I can fit my MS in the screen, I'm using a laptop. I tried what bleve said, playing on full screen, but that sorta makes the recording a bit harder and laggy, I'll still try it out next time.
