6 - 3 from potential, 2 from bonus potential (there's a cs thing which can increase number of bonus potential lines to two), 1 from neb

you can get up to 2 extra potential lines with this new system, 3 from regular potential, and the neb

I got one earlier today. It's a use item, 5% chance of working, if it fails 100% chance of blowing up your equip.

Edit: I blew up my time traveler's laurel. Oh well
