It's a pretty good bet that the mortality rate (for mothers and neonates) was a lot higher before the advent of modern medical knowledge.

Luck I guess. It would be funnier to imagine what the first pregnant woman thought was happening to herself.

well usually in the old days, death of the mother after childbirth was quite common.

There isnt too much medical knowledge that is needed when nature takes the reins on that process, of course it wasnt uncommon for females to die during child birth since they do loose a lot of fluids which werent replaced.
How? When there's pain in the lower abdomen the body's automatic response is to push.
