that's a hard question
I've never been visually disturbed by any anime/manga, I mean, there have been scenarios where I thought was totally f'd up. I think it's cus my mind process watching/reading the horror genre is just "lolfake"
in deadman wonderland, when crow's eye gets torn out of his eye sockets with no anesthetics and just a metal tong and some straps to hold him down
in another, when mizuno drops to her death in an elevator due to a snapped cable, all while on the phone with sakakibara
in higurashi, the nail pulling scene..... dear god, the nail pulling scene
I'm supprised Gakkou Gurashi hasn't been mentioned yet.
You guys should check it out, just bear with the first episode 'till the end

Ayy, I read Narutaru not too long ago. Very reminiscent of NGE.

eh.... I'm a bit of a weird/shock body horror/gore fan. Kinda why I like franken fran too. I like all the weird stuff that happens in his works (not necessarily uzumaki)

that one scene in psycho pass 2 with the surgically altered but still living bodies that were disguised as hologram animals burning up scarred me for life

Haven't watched it, but Victory Gundam is widely considered the darkest Gundam series to date. Yes, even darker than Zeta.
(Doesn't help that Tomino was incredibly depressed while directing it too, same with Zeta to a somewhat lesser extent)
honestly Uzumaki. it.. is just gross. plot sucks, art is meh, it's literally just gore gore gore.
Anything by Junji Ito
Anything from Kago Shintaro. I tried reading a few of his works... just wow. I'm down for weird stuff, but it went beyond the limits of what I'm okay with.
As for ones that I was actually able to finish (or read all that's available):
Tokyo Akazukin
Bradherley no Basha (... couldn't really enjoy this one)
Ookami no Kuchi: Wolfsmund
Higurashi. The parasites oozing out of the skin was kinda gross and the ripping of the fingernails was also disgusting
Another was sort of shocking like how the author kills all of them for example the funniest one of them all which was the umbrella stab
Ichi the Killer is pretty shocking in terms of goer. I can successfully say that it has caused me to wince quite few times reading it
despite that I found it quite alluring to read lmao

Is this the manga where he was in love with this prostitute? Man when that prostitute you know what I just straight up stopped reading cause the manga was just too much of a reality shock lolz.

Uzumaki is apparently really disturbing in the body horror sense, haven't read it though.
