I got all the nx items but i didn't get the item to open the totems. am i doing something wrong?

Unequip them, wait for a second, and then turn in the quest

You unequip (just as above) them for it to register on quest.

There should be a quest that gives you totem opener. Be sure to have all 4 of the NX acc. in your inventory to be able to complete the quest.
