Pretty sure SnS 3rd season didn't have a flawless plot. Tokyo Ravens had something missing, too, unless I'm mixing it up with the Light Novel. Haven't bothered finishing the first season on fairy tale.

A very tough question. I think it'd be a knife fight between Tengen Toppa Gurren Lagann and Clannad Afterstory. Death Note and Steins Gate would come really close though.
Well, on one hand, Cowboy Bebop is considered by most to be the "best" anime out there... though I think the strength of the series is more character-based than plot-based, since the series doesn't have a major overarching plot more than 5 or 6 episodes connecting over the series.
If you want an anime with a clever plot that works well on various levels, I'd go with Steins: Gate... I recently finished it and wow... the story is very well done.
It's all very subjective, even if you limit it to the best anime in a certain genre.
It's hard to define a "flawless" anime since what some people see as flaws might not be true to other people. An example would be that some people might think the pacing is a bit slow while others believe it to be just right. It also depends a lot on the genre; everything that makes a slice of life anime great wouldn't work in a action anime.

Wahhh, hunter x hunter was rly good for me until the chimera ant arc :O. I don't know why i didn't like that arc, but the rest is great.
Hunter x Hunter Chimera ant arc I would say is NEAR perfection, (dont think anything can be 100%, its always good to have anime top others, but its an arc, however its at the top of my list) a close second is Steins gate, (I didnt like the part where it was getting harem-ISH with the dating cycle he was going though (dont kill me on this ppl)
Since your asking for a complete anime I would give it Steins gate

There is no "perfect" anime.
Dont have a "perfect" anime, but Fate/zero comes pretty close.
