Blackheart got ridiculously buffed. Everything is the same, with one exception: He heals ~100 m hp every 1-2 minutes. Or something like that. It's a LOT of health. Me (hitting ~3m per demon impact), a buster hitting 1 mills on trinity, a bishop to resurrect, and another DS (who has half my range), and we couldn't whittle him past 1/3 hp because he'd keep healing.

okay, i just attempted, i finished 5th stage at 48:00
i got teleported back out instead of advancing into bonus stage

Yes, after the stage with statue in the middle that spawns skeletons. The times you listed are correct.
The blackheart boss isn't very hard, just learn its damage reflect and you should be fine.
