Let's say I fail 2 Golden hammers, can I recover those 2 slots with clean slates?

Yes. It will still say Hammers used (MAX), but each successful clean slate scroll will increase the Upgrade slot count by one.
That means you don't have to "re-hammer" to get the extra slot, but the formerly failed hammer slot becomes available for use with a scroll.
