I'm a UA so i have a few extra skills i had to put in.
Ctrl= Phantom Blow
Shift= Blade Fury
V= Final Cut
O= Asura
X= Buff (Haste + booster + karma)
C= Buff (Mirror Image + Thorn + MW)
F1= Hidden Blade
Ins= Buff (dSI + dHB + dAB)
pup= dCO
A= Chain of Hell
~= Flying Assaulter
Q= Bloody Storm

Not online now, but my layout is
a=FlyAs s=BloodyStrm d= UpwardS Shift=Tornado c=Jump v= Blade Fury and Ctrl = Chains of Hell
This format allows me to do FABSUS or FATSUS easily while keeping Blade Fury close by
Also, I don't use my right hand for attacking, just moving
