Happens when there's a lot of items on the ground, that you pets could potentially pick up.
Disable pet loot, it solves the problem.

Yes it happens whenever there's too many items on the ground.

That's not the reason....

Also you'll notice lag is worse on the ground level of the map and gets better as you go higher up on the map. The ladder will lag you a lot too

Item loot lag has been in hall of honor for a couple months now
