The description is absurdly poor. How it works: upon activation, you gain a small shield that gets increased to half your HP/99,999 based on your damage (which caps out easily). The shield itself acts as a ghetto damage reduction, with both your HP and your shield reducing for every hit taken.
@fradddd: The damage you'd take is split into your HP and shield, 50-50. *Doesn't affect HP% attacks When the shield is gone you take normal damage again.
Oh, this is a guess but I think the different color hp is the extra hp you gained from the buff. It's not your actual max hp but rather a temporary increase which is indicated by the different color.

When the shield is gone you take normal damage again.
@risingrain so does the shield protect you for half your HP, and after the shield is gone your actual HP starts draining?
You don't get any more HP. It just shows you how big the shield from Power Transfer is.
It caps out at 1/2 your HP, or 99,999, whichever is smaller.

Is it real HP? The numbers don't match up to the HP bar? Please help I can't afford to die to figure it out. Someone sacrifice themselves for me.
Yeah I couldn't figure that out either. Maybe you should ask one of the top ABs in the game @ashleyattacked
